Consent for the use of medicines not authorised/licensed in the treatment of horses or the treatment of a particular condition in a horse.

I am the owner of the above horse (or owners’ agent). I understand that South Moor Vets may, on occasion, and when necessary, use medicines which, while not specifically authorised/licensed (i.e have passed regulatory assessments for safety, quality and efficacy) for the treatment of this species or condition be considered justified and appropriate by the veterinary surgeon.
I understand that on each occasion South Moor Vets administer or prescribe an unlicensed medication for my horse, the vet will inform me they are doing so and there may be unknown side effects. I consent to the use of unlicensed medicines where they are clinically and legally justified and confirm that I am over 18 years of age.
